What is a Community Health Center?

A Community Health Center (CHC) works to improve the health of its community. CHCs provide medical, dental and mental health services and offer a sliding fee scale. The Lakes CHC is a Community Health Center.

What is the Sliding Fee Scale?

The Sliding Fee Scale is a discount based on your ability to pay. It can be applied to your bill at The Lakes. Insurance copays cannot be adjusted. Anyone who makes less than 200% of the Federal Poverty Level is eligible.

How do I apply for the Sliding Fee Scale?

You need to complete an application and provide proof of income or no income. Read more about applying for the Sliding Fee Scale.

Are you a free clinic?

No, but we do make health care more affordable. We accept insurance, and uninsured patients are billed based on their ability to pay.

Who can go to the Community Health Center?

Anyone, from anywhere, can come to The Lakes.

Can The Lakes help me with prescription medication discounts, housing, FoodShare, BadgerCare, and Disability even if I’m not a patient at The Lakes?

Yes, The Lakes will help anyone with these services. You do not need to be a medical or dental patient to get help from a Benefit Counselor.
